Guidance on implementing the Nursing Home Survey is still under development. If you are interested in administering this survey, please contact us and we’ll work with AHRQ to develop an appropriate study design.
Survey Instruments
- Long-Stay Resident Survey – An in-person structured interview for long-term residents.
- Discharged Resident Survey -A mail questionnaire for recently discharged short-stay residents.
- Family Member Survey -A mail questionnaire for residents’ family members
